At the request of numerous medical providers across the globe, SHAPE
and J & J Medical have developed an Internet hosted, self-paced
certification program for physicians and non-physician providers
(nurse practitioners, physicians assistants, nurses).
SHAPE Certification addresses the needs of: Cardiologists,
Radiologists, Internists, Family Physicians, Primary Care Providers,
OB-GYNs, Preventive Medicine Specialists, and Non-Physician
Providers.

Educational components are presented in a user friendly video module
format from the Society for Heart Attack Prevention and Eradication
(SHAPE) and world recognized faculty on CVD risk assessment and
early CAD detection/ heart attack prevention strategies.
Specific educational modules include: Appropriate patient selection
for CAD screening, risk assessment for subclinical CAD, coronary
artery calcium scoring, carotid IMT testing, understanding the
clinical and economic burden of global CVD, comparison of available
screening tests, and how to use the SHAPE Guidelines to improve
patient outcomes.
Following completion of self-paced educational modules (8 Modules:
Total Running Time 148 minutes), applicant will complete a
web-hosted exam. Certification for those successfully completing
exam.
Certification Program Instructions
These simple instructions will guide you through the SHAPE
Physician/Provider training and certification process.
Step 1: Click on the link below to begin the educational
modules. Note: You may stop and return at any time until you have
completed all of the educational sessions. You may also repeat any
modules, at any time, to increase your understanding of the
material.
Step 2: When you have completed all 8 educational modules and
understand the contents click on the link labeled "Take On-Line
Exam".
Step 3: Complete the multiple-choice exam and follow
submission instructions at the bottom of the exam page. You will be
promptly notified if you have passed the certification exam (>70%).
If you successfully pass the certification exam you will be invoiced
the one-time SHAPE Certification fee ($375 Physicians, $250 Nurse
Practitioner/Physician Assistants, $200 Registered Nurses). If you
did not pass the exam you will be asked to view the educational
modules again and then re-test.
Step 4: Once your SHAPE Certification fee is received you
will be awarded you certificate, have your name added to the SHAPE
Certified Provider webpage, and receive your CME certificate along
with other materials from SHAPE as described above.
